Faculty/Staff Permit Reactivation or Cancel FormSubmit this form if you are a WashU employee and would like to reactivate or cancel a parking permit that is currently on hold.
Permit Hold and Cancel FormSubmit this form if you are a WashU employee and want to place your parking permit on hold. If you are a student or BSC, fill out this form to request a permit cancellation. 
First Year Exception RequestFill out this form if you are a first year student and need to request an exception that, if approved, will allow you to park on campus.
Medical School Parking Permit Reciprocity Request FormThis form is for WashU Medical Campus permit holders with reciprocity who would like to park on the WashU Danforth Campus.
Parking Citation AppealTo contest a parking violation, submit this form within seven calendar days of the violation.
